HEALTH AND WELFARE PLANS

Medical Plan: Medical insurance represents the largest aspect of the sanofi pasteur Benefits Program, offering a premium health plan to employees and eligible dependents. The Comprehensive PPO Plan is designed to provide quality health care with low out of pocket maximums and co-pays. The program gives employees and family members the freedom to receive care from many resources and covers a broad range of health care services and supplies, including office visits, hospitalization and supplies. In addition, employees are able to select any physician and not have to choose one Primary Care Physician.

Prescription Plan: By enrolling in the company-provided medical plan, employees are automatically a member of the prescription plan.  Prescriptions are paid at 100 percent after a small co-pay. Additionally, Sanofi Aventis prescriptions are free of charge to plan participants. For added convenience, all employees and their family members are eligible for our mail order program.

Dental Plan: The company-provided dental plan allows participants to visit any dentist; however, by obtaining services from a dentist who participates in the provider network participants will receive a higher level of benefits and thereby reduce out-of-pocket cost.

Vision Care Plan: The company-provided vision plan offers a preferred provider plan with a network of providers and optical suppliers. As with a medical or dental PPO, using the network of providers in the vision plan reduces out-of-pocket expenses and eliminates the need to file claims.

Flexible Spending Accounts (FSAs): FSAs provide tax advantages by allowing participants to set aside pre-tax dollars for eligible expenses. The company offers both Health Care and Dependent Care Flexible Spending Accounts (FSAs). A Prepaid Visa card is provided for the Health Care FSA allowing for immediate payment without having to submit claim forms, or wait for reimbursement checks.

Company-Provided Life Insurance: The company provides Basic Life Insurance and Business Travel & Accident Insurance at NO COST to participants.

Supplemental Life Insurance: In addition to the company-provided insurance coverage, participants may purchase Voluntary Life/Accidental Death and Dismemberment Insurance.*  The company also offers Supplemental Life Insurance for employees looking for additional security for spouses and dependent children.

Short-Term Disability (STD) and Basic Long-Term Disability (LTD) Plans:: This coverage may replace some or all of participants’ income in the event of an illness, injury or disability. Additionally, employees are offered Voluntary LTD; a buy-up option that allows employees to purchase a higher level of LTD coverage beyond the employer-paid basic amount.*

* Subject to Evidence of Insurability rules
